Planned Parenthood of Orange and San Bernardino Counties has a full-time opportunity for a Director of Case Management in Anaheim, CA.
Oversee and manage the Case Management Department, which provides follow-up of abnormal labs/diagnostic studies and external care referrals in accordance with Planned Parenthood regulations and protocols and the Board of Registered Nursing. Oversee and manage the health center Patient Navigators who provide onsite support to patients and members of the primary care team before, during, and after primary care appointments.

Qualifications

Licensure and/or Certification Requirements:
- Minimum licensure requirement is an active California Registered Nurse’s License.
- CPR certification required.
Minimum Education:
- AS/BS in Nursing required.
Minimum Work Experience:
- 1 year of clinical experience required.
- 5 - 8 years of case management experience preferred.
- Experience in value-based healthcare model required
Supervisory Experience:
- 5 - 9 years of supervisory experience preferred.
- Demonstrated ability as an effective leader, including coaching and team-building skills.
- Good managerial skills, which require the ability to effectively manage administrative functions for the Department.
- Strong written, verbal and interpersonal skills.
- Budgeting experience.
- Excellent strategic planning skills.
- Must be self-motivated and have the ability to work without direct supervision.
Other Requirements:
- Ability to work independently and multi-task.
- Ability to work under standardized protocols and procedures.
- Excellent interpersonal, problem solving and communication skills.
- Ability to physically assist a patient in distress.

- Must demonstrate high-level computer skills including; Microsoft Word, Excel and Outlook. Electronic medical records experience may also be required.
- This position requires travel to other sites and locations; if using a personal vehicle to meet this requirement, a valid CA driver’s license and current auto insurance in compliance with the minimum requirements of CA vehicle code are required.

Responsibilities
