About the role

What You’ll Be Doing
The Aging Well Conflict Free Case Management (CFCM) Supervisor oversees case management services under the CFCM Model for adults with intellectual or developmental disabilities and adults aged 65 and older. This role ensures individuals eligible through EOHHS, BHDDH, and DHS receive person-centered, high-quality, and timely services. In coordination with the Program Director, the Supervisor provides leadership, trainings, and support to case managers, ensuring program goals and quality standards are met across Rhode Island.

Hybrid Role, 20% Remote Work. 

Your Main Priorities
While additional responsibilities are likely to arise on the job, you will primarily:
•    Establish and implement program policies, goals, objectives, and procedures, conferring with department/organization leadership and staff members, as necessary.
•    Oversee the development and implementation of quality and performance initiatives for the program in collaboration with Performance and Quality Improvement and department Director.
•    Be a specialist in current issues and local services for the program participants in order to provide information and referral to the community.
•    Actively promotes the Aging Well program to community service agencies and health facilities to increase public awareness, financial support, and referrals.
•    Provide short-term counseling and assessment to homebound participants to facilitate problems related to loss, failing health, isolation, and loneliness.
•    Act as an advocate for the participants by connecting them to the services and support systems needed.
•    Develop programs, workshops, and support groups to inform and train the participants to use their own resources with support from the Training Coordinator and Human Resources.
•    Lead workshops and staff training to educate professionals and families about special needs and issues for the participants.
•    Consult with families of participants to offer information, support, and referral to aid decision making and establish peace of mind.
•    Is an active member of aging and disability organizations by contributing to the community effort to resolve participant issues and concerns.
•    Assist the Director in the recruitment process to fill vacant positions with qualified candidates.
•    Assess staff needs, including number of staff, skill sets, and appropriate schedules and budgets; accordingly, assumes case management responsibilities as needed. 
•    Provide individual and group supervision to case managers within the program.
•    Perform ongoing performance assessment of employees; including home visit observations, and complete annual performance reviews on a timely basis, as assigned.
•    Supervise employees in a proactive fashion to sustain or improve job performance; provides corrective action that is fair, objective, timely, and developmental.
•    Assess and analyzes competency patterns and learning and educational needs of staff, based on supporting data.
•    Develop an ongoing department education and training plan that encompasses safety issues, competency, and continual improvement processes, which can be linked to department goals.
•    Develop and implement program policies and procedures and ensure adherence; supports department leadership with reports and other contractual obligations.
•    Attend ongoing program development meetings, training, and networking opportunities.

Who You Are
Requirements, Skills & Abilities:
•    Associate degree in social work, human development, Gerontology, or related field required; bachelor’s degree in social work, Gerontology, or related field preferred. 
•    Minimum 4 years’ experience working with elders and aging population or adult with intellectual and developmental delays required; experience in case management preferred. 
•    Excellent organizational, project management, and time-management skills.
•    Strong interpersonal communication and collaboration skills.
•    Strong organizational, critical thinking and time-management skills.
•    Basic computer skills required; knowledge of Microsoft Office 365.
•    Ability to communicate and effectively interact with a diverse population.
•    Bilingual preferred. 
•    Collaboration and teamwork. 
•    Leadership. 
•    Must have a valid driver’s license, proof of auto insurance and car registration. 
•    Must authorize Child & Family to complete a motor vehicle records check.
•    Must meet all required background checks and medical clearances.

Physical Requirements
Direct Service Staff:
This position requires time sitting, standing, walking, driving, carrying and lifting up to 25lbs. This role also requires operating desktop, laptop
