Details: Full-time with benefits, working 40 hours a week. This position will work Monday- Friday, 8:00am-4:30pm. No weekends or holidays!
Key daily responsibilities of this role:
Assisting other team members of Integrated Care Management prepare patients for their discharge plan Talking to patients and family members about needs following discharge

Position Summary:
Consults with all necessary stakeholders to train and educate on various aspects of the Integrated Care Management process in order to achieve top quality results. Supports Integrated Care Management team to ensure appropriate discharge practices are carried out including but not limited to referral placement to home and extended care facilities. Complies with CMS guidelines by timely delivery of needed documentation. Assists in decreasing readmissions by ensuring post discharge primary care physician appointments are secure. Supports and monitors the outcomes of post-acute vendors with timely follow-up of patient concerns and monitors quality data in alignment of the IU Health Collaborative.
Position Requirements:
• Requires 0-3 years of related experience.
• Strong verbal and written communication skills are required.
• Advanced working knowledge of Microsoft Excel, Word and PowerPoint software applications is required.
• Requires excellent organizational and project management skills and ability.
• Must have strong analytical skills and be an accomplished problem-solver. Bachelor's Degree or equivalent years of experience required.
