Overview

Coordinated Care Agency offers comprehensive services, enhanced shelter, and housing for people experiencing poverty and homelessness.  Our services include permanent supportive housing, enhanced shelter, rapid re-housing, tenant-based voucher programs, outreach and engagement services, day centers, and health services. The purpose of our work is to ensure people have access to the housing, healthcare, and support they need to thrive. The team works to promote social changes to dismantle systemic racism, injustices and inequities that contribute to housing instability.

We operate in the spirit of the ministry of presence, fostering individualized attention and respect for each person’s unique circumstances. Our programs serve highly vulnerable individuals who may have significant needs, including severe physical disabilities, chronic or acute mental illness, active chemical dependency, and experiences of trauma. We focus on helping participants obtain or maintain permanent housing through consistent, supportive case management. This position oversees multiple programs. All programs provide housing case management, move-in assistance, and various methods of ongoing housing support for families experiencing homelessness.

This position would be responsible for the daily employee, client, and fiscal health of the programs. This will involve ongoing supervision and training for staff, addressing client concerns, ensuring model fidelity, and invoicing program expenses. This position would directly supervise 2-3 Services Managers and two Case Mangers. Indirectly this role supervises 5-6 Case Managers and one Lead Case Manager. This position would be responsible for contract compliance and maintaining budgetary mandates.

 

This position offers a compensation range of $35.75 – $39.77 per hour (DOE)

Responsibilities

Leadership

  • Provide leadership to programs and support the overall department culture. This position is a key contributor to the mission and vision for services in Short Term Housing Stabilization (STHS) Department.
  • Oversee the hiring and supervising for their programs and support the department through the internal Hiring.
  • Provide direct supervision upholding Organizational and Department Values and supporting staff growth.
  • Create processes and relationships which encourage collaboration and accountability to contract compliance.
  • Step in to support the duties of the Program Director in their absence.

Program Operations

  • Implement a quality improvement process that includes: data quality, contract compliance, file reviews, HMIS management reports and other system management reports and make recommendations to improve service delivery to clients, provider community and funders.
  • Collaborate with internal and external partners to provide high level services to clients. This will include facilitating care coordination.
  • Ensure client records are kept in accordance with agency standards amongst their programs.
  • Ensure that all agency required practices such as Credit Card Expenditure Forms, Timesheets, and HR needs are attended to in a complete and timely manner
  • Pursue system minimum standards which include exiting at least 80% of households to stable housing.

Fiscal

  • Review, approve and reconcile client assistance expenditures of approximately $450,000 annually.
  • Prepare and submit invoices to contract funders.
  • Evaluate and adjust program expenditures to ensure appropriate contract spending.

Building Operations

  • Coordinate with multiple programs to ensure safe and smooth operations at organization properties.
  • Coordinate with IT to address any program wide issues.
  • Design and oversee practices to support client service and staff support.  Set a positive client centered tone in the space.
