WeCare is looking for a motivated and talented Regional Clinical Director SNF LTC!

Organizational Contribution     Assist SNFs through auditing Data and communicating with leadership at facility level to improve resident outcomes, achieve benchmarks, and drive organizational goals. Regional RN utilizes clinical expertise and judgment to improve clinical practices within the organization through innovation and evidence based research. Regional RN assists organization on aligning facility practices with overall goals and providing direct support to facilities during emergency/ day to day operations.

Key Responsibilities

*Quality Metrics/ National Benchmarks monitoring*Survey Deficiencies/trends in reportable incidents*CMI Scores/trends in collaboration with reimbursement specialist*PDPM Performance and capture of clinical acuity in collaboration with reimbursement specialist*CASPER Reports*Compliance with CMS Regulations in collaboration with VP Nursing
-Oversee facility’s timely completion of reportable events investigations, Plan of Corrections and oversees reports prior to submission to DOH-Ensures SNFs maintain access to reporting channels as required by DOH/CMS-Ensures completion of pertinent regular Facility Clinical Systemic Audits-Maintains educated on Regulatory compliance; changes in local & federal regulations through alerts/Association Membership Memos etc-Attends LTC Association Education Conferences/Webinars etc-Performs or Arranges Mock Surveys for SNFs as needed-Recommends changes/updates in EMR to maintain compliance with Federal and Local Standards of Practice-Performs GAP Analysis on identified areas of focus-Performs SITE VISITS during: Local DOH Annual Recertifications Surveys/Federal Surveys , Reportable incidents that may rise to level of IJ, & Changes of DON/ADONs/ADMINs. -Regular Site Visits to assigned facilities-Assists in hiring /screening candidates for clinical leadership roles-Assess strengths and weaknesses of ea. facility DON and creates plan for improvement-Proponent of upward mobility/encourage Certifications/Specializations/ and continuing education for direct care staff-Ensures Weekly Meeting held with DON’s & VP Nursing-Holds educational seminars/webinars on “hot topics”/identified trends in their specific region-Provides educational resources to SNFs as needed; Assist facilities to identify barriers in QAPI-Attends resident council meetings regularly as invited-Assist SNFs Transition to 100% EMR -Streamlines policies 

Skills, Knowledge and Expertise

Current RN License in the State of PA
*Previous experience as Regional RN required
