We are searching for a Director of Care Transitions (Social Worker). This position is crucial for helping to drive the Ignite culture with the rest of the facility and will partner with the General Manager and leadership team to bring our vision to life. The ideal candidate will have knowledge and experience in social work, discharge planning and a passion for dealing with the senior population and working in and with Skilled Nursing Facilities.
