Compensation Range:

$37.00 - $55.50 per hour, depending on experience

Job Responsibilities:

As a key member of our multidisciplinary care team, the Diabetes Nurse Educator plays a vital role in supporting patients across the care continuum through education, intervention, and clinical coordination. Responsibilities include:

  • Triage and assess patients for early signs and symptoms of diabetes-related complications to ensure timely intervention and prevent adverse outcomes

  • Educate patients and families on diabetes self-management, lifestyle modification, and disease prevention using evidence-based resources

  • Develop and lead individual and group education programs following American Diabetes Association (ADA) guidelines to support long-term health outcomes

  • Monitor and evaluate the effectiveness of educational interventions through patient feedback, glycemic control trends, and documented health outcomes

  • Provide instruction and support for the safe use of diabetes-related medications and devices including insulin, injectables, glucagon, glucometers, insulin pumps, and continuous glucose monitors (CGMs)

  • Review and interpret data from blood glucose logs, insulin pumps, and CGMs to assist with care planning and provider collaboration

  • Perform insertion and download of CGM sensors for review by providers and assist patients in troubleshooting device-related concerns

  • Coordinate patient-centered care by collaborating with families and the interdisciplinary team across inpatient and outpatient settings

  • Work closely with the Unit Coordinator to support compliance with Joint Commission, NYS Department of Health, and infection control standards

  • Promote diabetes awareness and education by participating in health fairs, community outreach events, and organizational wellness initiatives

  • Support ADA credentialing and quality initiatives for the UHS Diabetes and Endocrinology Center by maintaining standards and documentation

  • Stay current with clinical best practices in diabetes education and care through ongoing professional development and continuing education

Position Qualifications:

Minimum Required:

  • Graduate of an accredited school of registered nursing.

  • At least 2-year experience in health care setting with evidence of having obtained proficiency in those areas emphasized in the Registered Nurse curriculum.
  • Bachelor’s Degree in Nursing.
  • Current NY State RN license.
  • Documentation of current CPR certification required within 1 week of start of employment.
  • Certified Diabetes Educator within 2 years of hire.

Preferred:

  • Previous experience in a walk-in/primary care/specialty care setting.
  • Current Driver’s license.

  • Office specific- IE: Oncology Certification, Lactation Consultant, Wound/ Ostomy Certification.
