SUMMARY

The Senior Program Manager-Care Management is responsible to monitor, supervise and coordinate delivery of quality clinical services throughout the Rochester Regional Health system through the documentation and reporting of data, as well as monitoring progress and tracking variances to ensure the organization continues to develop initiatives that have an impact on future sustainability.

RESPONSIBILITIES

• Ensures that the systematized programs meet the needs of consumers through an ongoing process of development that includes assessment, planning, and the implementation of plans.
• Manage expectations and the delivery of outcomes; aligning processes with Strategic and Management Plans for each hospital within the system.
• Reporting of data and status of processes, projects and programs at regular intervals as set-forth by leadership.
• Ensures that expenses which are delegated as a program responsibility are within the limits of the budget.
• Develops and implements a plan for the recruitment, hiring, orientation, and ongoing development of staff that ensures a continuity of quality and provision of services.
• Ensures that policies and procedures of the program are in compliance with all regulatory and hospital standards and that staff at each location understand and implement said policies and procedures in a consistent fashion.
• Manages the day to day operations of the programs which includes conducting meetings at all sites, providing leadership to the quality improvement process, promoting open communications and resolving day to day issues and concerns of consumers and staff.
• Ensures effective communication within the programs, the departments, the hospitals, and in the community.
• Evaluates direct report staff performance, documents performance review, and meets regularly with staff to assure compliance and staff satisfaction.
• Participates in peer review process and utilizes quality improvement measures to increase quality and reduce cost.
• Works in conjunction with multiple leaders throughout the organization.
• Performs other duties as assigned.

REQUIRED QUALIFICATIONS

• Must be enrolled as a matriculated student in a bachelor-prepared accredited college and scheduled to graduate within one (1) year of appointment to position.

PREFERRED QUALIFICATIONS

• Associate’s Degree; Bachelor’s Degree preferred.
• Two (2) years of supervisory experience.
• Excellent organization, communication, interpersonal and problem-solving skills.

$60,500.00 - $84,500.00

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
