Provides education about rehabilitation services to physicians, social workers, discharge planners, insurance companies, case managers and rehabilitation case managers to expedite patient referrals. Assures safe, appropriate, efficient and effective admission of patients into the rehab unit by performing clinical assessments of patients referred to the rehab unit. Assures completion of the intake assessment of inpatient unit.

Qualifications

In addition to bringing humankindness to the workplace each day, qualified candidates will need the following:

 

Associates Degree in Nursing (ASN), or Bachelors Degree in Physical Therapy, or Bachelors Degree in Occupational Therapy, or Masters Degree in Speech Therapy with Certificate of Clinical Competence

Bachelors Degree

Masters/Doctorate Degree in Physical Therapy, preferred. Masters Degree in Occupational Therapy, preferred

2 years of rehabilitation or other appropriate post-acute environment experience1 year of proven marketing experience demonstrating a consistent ability to achieve objectives, preferredExperience/knowledge in community health setting/services, preferred

Current license in the state of practice from one of the following: RN license/ RN license from a participating state in National Licensure Compact, or licensure in Physical Therapy, or licensure in Occupational Therapy and registered Speech Language Pathologist or Certificate in Competence from the ASLHA.

BLSCertified Rehabilitation Registered Nurse (Preferred)

 

Sedentary work - prolonged periods of sitting and exert up to 10 lbs. force occasionally

 

This PRN (as needed) role works at least 1 weekend per month and covers PTO needs during the week for our inpatient team at St. Anthony Hospital.  We are looking for a RN, PT, OT, SLP who has previous clinical navigator/liaison experience.
