MDS Coordinator (RN) - Float Position Supporting the Louisiana Region

Role Overview

As an MDS Coordinator, you will work closely with an interdisciplinary team to assess residents’ needs and optimize their care plans.

Key Responsibilities

• Participate in the pre-admission process, ensuring essential information for MDS optimization is obtained

• Review resident care and set ARDs to ensure accurate Prospective Payment System MDS Assessments

• Maintain compliance with state and federal regulations for timely and precise resident assessments

• Accurately code MDS data based on interviews, medical records, and direct observations

• Manage MDS schedules and assist with additional assigned duties

Required Qualifications

• Active RN license in Louisiana or ability to obtain

• Prior experience in MDS coordination

• Strong interpersonal communication and teamwork skills

• Familiarity with the RAI process and Case Management

• Ability to travel extensively throughout Louisiana to support multiple locations
