Job Location Developmental Pathways Aurora Office - Aurora, COPosition Type FT - 40 hrs/weekEducation Level BachelorsSalary Range $24.44 - $26.88 HourlyJob Shift DayDevelopmental Pathways (DP) has an opportunity for a Senior Case Manager (SCM) – Active in our Case Management Department. The Active SCM will be responsible for verifying eligibility and developing, monitoring, and assuring implementation of the Service Plan (SP). The Active SCM will manage a caseload of individuals receiving waiver services and maintain documentation needed quarterly and annually. The Active SCM will facilitate interdisciplinary teams (IDT) meetings to coordinate services to meet individual needs; ensure appropriate interaction and communication; and implement IDT meetings as appropriate to modify services. Lastly, will provide case management services and support to temporarily assigned individuals; support with department and team coverage as needed and provide training and mentorship for Active Case Managers. This is a 40-hour per week, hybrid position, with the benefit of working remotely and in-person coming into our headquarters office in Aurora, CO and/or off-site locations.

A Bachelor’s Degree,
- OR Five (5) years of experience in the field of Long-Term Services and Supports (LTSS), which includes Developmental Disabilities;
- Or some combination of education and relevant experience appropriate to the requirements of the position
- Must have demonstrated exemplary performance while working within service coordination or case management positions, including the following areas of performance: demonstrated initiative within the department or agency on projects, demonstrated leadership skills, adaptation to change, and problem-solving skills.
- Valid Colorado motor vehicle license & proof of motor vehicle insurance
- Ability to meet and maintain agency driving requirements
