The Road Home has been a leader in the fight to end homelessness for 100 years (1923-2023) . We provide low-barrier emergency shelters, supportive services, and housing-first-minded rental assistance that helps individuals and families step out of homelessness and back into the community. Job Summary
The Veteran’s Case Manager will work to ensure that any veteran living in homelessness in Salt Lake County is engaged and has access to housing resources. This position will ensure that men, women and families who have served our country and are living at The Road Home, on the streets or in a partnering shelter are supported as they overcome homelessness. The goal of the SSVF program is to provide low-income veterans experiencing homelessness or at-risk of experiencing homelessness with a range of supportive services designed to promote housing stability. The Veteran’s Case Manager will work to identify common barriers faced by veterans and establish strategies and procedures for addressing needs on an agency-wide scale

Requirements
JOB DUTIES
Housing First
- Help Veterans experiencing homelessness get into housing.
- Assess barriers to housing including helping guests get access to vital documents.
- Moving guests and their belongings to their new homes. *
- Use agency vehicles to provide transportation for guests. *
- Strong belief of the Housing First model. Willingness to initiate and have ongoing conversations with guests about housing.
- The ability to engage and encourage housing conversations with guests
Managing a Case Load
- Provide direct case management, including crisis intervention, assessments, goal setting, problem solving, agency and community resources utilization, and network building.
- Working with our diverse guests on your set caseload to set housing goals
- Engage with all guests during daily open office hours to connect them with resources
- Setting healthy Boundaries with guests
- De-escalate, empower and advocate for the diverse populations we work with.
- Daily collaboration with shelter staff
- Seeking out and engaging with community partners to find housing options.
- Attended weekly staff/staffing meetings
- Keep records, case management notes, and statistical data as required and participate in research and/or special projects as needed.
Promoting Best Practice
- Demonstrates awareness and desire to understand diverse populations.
- Ability to contribute to the Agency's commitment to enhancing awareness and appreciation of diverse ethnic and cultural heritages.
- Willingness to understand that housing first, harm reduction, low barrier access to resources is best practice and essential to our mission at The Road Home.
- Ability to set and maintain good boundaries, using Trauma-Informed Care as a guideline

Education and Experience
- Lived, work or volunteer experience preferred or a degree in a related field
- Experience working or volunteering with diverse populations
Required Skills and Abilities
- Strong interpersonal skills and ability to work with diverse populations.
- Demonstrated awareness of and sensitivity to diverse populations. Ability to contribute to the agency’s commitment to enhancing awareness and appreciation of diverse ethnic and cultural heritages.
- Excellent writing and documentation skills.
- Willing to be a proactive member of a team.
- Ability to work from a Trauma-Informed Care framework.
- Familiarity with the Housing First philosophy.
- Ability to work with community partners.
- Self-starter and self-directed.
- Ability to accept direction from Supervisor.
- Excellent organizational skills and strong computer skills.
