Position Summary:
Children's Hospital Primary Care Center (CHPCC) and Primary Care at Martha Eliot are here to help families raise healthy, happy children. Our locations serve more children than any other primary care practice in Boston.
As a Population Health Assistant, you will coordinate patient scheduling, population reporting, and related support activities and functions for patients in our asthma and nurturing wellness programs in primary care.

Key Responsibilities:

  1. Make outreach to patients, schedule appointments, and engage with clinical staff to ensure appropriate care is provided.
  2. Work with clinicians, administration, and other facilities, such as hospitals and public health agencies, in order to coordinate the collection of data for each population and track this information.
  3. Prepare monthly or quarterly reports on population activities, progress, status, or other special reports, as assigned, to present to the clinical team.
  4. Draft written communication and promotional literature as part of the population management outreach efforts.
  5. Interact with and maintain liaison with staff, faculty, and outside/community agencies in facilitating program objectives.
  6. Provide direct administrative support in obtaining documentation and other materials for patient care.
  7. Manage visit requests from hospitals discharging patients to BCH and initiate internal communication regarding discharge and follow-up status of high-risk patients in other hospital departments.

Minimum Qualifications
Education:
  1. High School Diploma/GED.
Experience:
  1. 1 year of experience in customer service or clinical research assistant work.
  2. Bachelor’s degree may substitute for one year of experience.
