LSSI is hiring a full-time Substance Use Outpatient Counselor Case Manager. Provide individual, group, and family counseling services for clients experiencing SUDs and substance- related issues in a clinical, culturally sensitive, and trauma-informed manner for our Elgin Outpatient substance use treatment program. Schedule: Tuesday through Friday from 10:00am to 6:30pm and Sunday from 9:00am to 5:30pm Benefits and Perks:

- Provide a continuum of counseling services to individuals with alcohol/drug, co-occurring, or gambling related conditions and/or problems using a variety of therapeutic modalities and evidence- based practices.
- Coordinate and facilitate group and individual counseling sessions (ASAM Levels 0.5, 1, 2.1, continuing care), DUI, and psychoeducation.
- Responsible for an assigned caseload and the associated documentation including treatment planning, insurance authorization, continuing care, case management and discharge plans. Monitor progress and participants’ response to treatment.
- Communicate professionally and consistently with referral sources to provide status reports and completion/discharge documentation for mutual clients.
- Complete biopsychosocial assessment, alcohol/drug evaluations, and DUI evaluations using DSM- and ASAM criteria.
- Maintain weekly performance productivity.
- Ensure adherence to ethical standards of treatment, including NASW Code of Ethics.
- Demonstrate professional, positive behavior and carry out responsibilities with integrity, treating clients, families, other LSSI workforce members, and collaborative organizations and/or individual I a dignified, respectful, honest and fair manner.
- High school diploma or equivalent required.
- Bachelor’s or master’s degree in social work, psychology, counseling or related field strongly preferred.
- One (1) to three (3) years of experience in substance use field preferred.
