Physical Therapist (PT) – Home Health | Mission Home Health
Mission Home Health is seeking a skilled and compassionate Physical Therapist (PT) to join our team in the South Scottsdale and surrounding areas. This role offers the opportunity to make a meaningful impact by providing high-quality care to patients in their homes, helping them regain mobility and independence.
Position Details:Compensation: SOC: $115-$120Revisits: $80-$90Evals: $90-$100Hourly Rate/NVA: $35-$40 (education and training)Employment Type: Full-time, Monday-Friday, 8AM-5PM (we are flexible on scheduling!)Service Area: South Scottsdale / Paradise Valley
Responsibilities:
- Evaluate and assess patients' physical conditions and develop individualized treatment plans- Implement therapeutic exercises and techniques to improve mobility, strength, and coordination- Educate patients and caregivers on exercises, safety precautions, and self-care strategies- Collaborate with an interdisciplinary team to provide holistic, patient-centered care- Maintain accurate and timely documentation of patient progress and treatment plans
Requirements:
- Active Physical Therapist license in the state of Arizona- Doctorate or Master’s degree in Physical Therapy- Minimum of one (1) year of experience as a Physical Therapist (home health experience preferred)- Current CPR certification- Reliable transportation with valid driver’s license and auto insurance
