The Senior Care Management Support Professional (PDACS) supports the Participant-Directed Attendant Care Services process which empowers Members, or their representatives, to manage their own personal attendant care.- Engage and educate internal and external stakeholders about the concepts of PDACS and what is required to successfully develop and deliver service within the paradigm
- Analyze, monitor and provide periodic reports on the utilization of PDACS. Utilize the data to inform strategies and future work.
- Assist in developing, implementing and evaluating internal policies, procedures and guidelines for services
- Support Care and Service Coordination by informing of approved or denied service workers and collaborate as needed
- Follow up on any referrals pending action
- Supports investigation, closure and data sharing for grievances & appeals, quality of care, and FSSA escalations
- Tracks and trends member dis-enrollments and assist with member transition between MCEs
- Support and participate JOC meetings quarterly
- Complete state mandated reporting as required
- Partner with market training to provide content for ongoing training, education and hands-on learning opportunities to partners, all levels of member support management, and other divisions to support the development of organizational competencies
- Facilitate a cycle of continuous improvement to assess and address performance gaps in utilization of PDACS
- Potential for travel within the state of Indiana

Required Qualifications
- Must Reside in Indiana
- Bachelor's degree in healthcare, public administration, nursing, social work or other related field or equivalent education and experience
- 3+ years of technical and/or business project management experience
- Technical experience with Microsoft, Word, Excel, Outlook, knowledge of Microsoft Word, Excel and Outlook
- Understanding of and strategic thinking and business planning capabilities; organized and detail-oriented
- Excellent written and verbal communication skills
Preferred Qualifications
- Experience in Health Plan Operations, Healthcare, or Long-Term Care business systems
- Six Sigma and/or Project Management Institute certification
- Experience working with Medicaid and/or Medicare Plans and consumer directed services
