Job DetailsJob Location: Corner House - New York, NY 10030Salary Range: $55,000.00 - $60,000.00 Salary/yearIntro/Program Description: Goddard Riverside’s Corner House provides affordable, permanent supportive housing for 34 previously homeless adults many of whom have mental illness, substance use, and other co-occurring disorders. On-site services include case management, entitlement assistance, medical and psychiatric care, substance use counseling, group activities, recreation, and 24 hour/7 day per week front desk security.
Position Summary: To deliver effective case management services using a strengths-based, client-centered approach to formerly homeless adults diagnosed with a severe and persistent mental illness to maintain permanent housing. The Senior Case Manager will provide administrative support to the program director with day to day services/ operations while carrying a caseload.
Schedule: Monday-Friday 9:00am-5:00pm (flexible)
Salary Range: $55,000-$60,000
Candidates must have the required qualifications to be considered for the position. The salary offer will be based on the candidate’s number of years experience in the exact or comparable position and additional preferred qualifications will be taken into account.
Roles, Responsibilities, and Essential Duties Roles, Responsibilities and Essential Duties
1) Client service
Provide case management services to an assigned caseload of 20 clients, including identifying short- and long-term treatment goals and assisting clients in developing and achieving individualized service plans.
Link clients to appropriate community resources and services, including medical, psychiatric, substance use treatment, housing, legal, and other social service programs.
Assist clients with applying for, obtaining, and maintaining eligible benefits and entitlements.
Assist clients in understanding and complying with building rules, policies, and procedures to support housing stability and maintain residency.
Maintain accurate, timely, and comprehensive case records documenting services provided, client progress, interventions, and outcomes in accordance with agency, regulatory, and funding-source requirements.
Complete case notes, service plans, assessments, and other required documentation using the agency's electronic database.
Lead and coordinate meetings with clients, case management staff, and building management as needed to address housing, service, and tenancy-related concerns.
Coordinate and track medical, psychiatric, and other healthcare appointments between clients and healthcare providers.
Accompany and escort clients to appointments, community activities, and outdoor events when necessary, taking the tenants on trips including providing support during periods of reduced staffing.
Provide culturally responsive crisis intervention, advocacy, supportive counseling, and service coordination to clients.
Develop and maintain professional relationships with healthcare providers, community organizations, legal services, and other service providers to support client needs.
2) Administrative Duties
Assist the Program Director with the orientation and training of newly hired social service staff.
Conduct monthly case record audits and provide feedback regarding documentation compliance and quality.
Participate in tenant intake and admission meetings and assist with the assessment and transition of new tenants into the program.
Attend external meetings and develop and maintain community linkages in the absence of the Program Director.
Serve as the senior staff member and provide operational support in the Program Director's absence.
Assist with the coordination of program activities, community referrals, and supportive services.
Provide guidance, consultation, and support to members of the case management team.
Participate in ongoing professional development and educational opportunities and share relevant educational resources with staff.
Develop and maintain ongoing referral relationships with partner organizations and providers within the healthcare, legal, behavioral health, housing, and social service systems.
Provide advocacy and support to clients and assist staff in addressing complex client needs and service barriers.
3) Corner House team member responsibilities
Participate as an active member of the Corner House team.
Attend staff meetings, case conferences, training programs, and individual and group supervision meetings.
Attend Incident Review Committee meetings in the absence of the Program Director.
Collaborate with staff to develop, maintain, and regularly update an active community resource directory.
Conduct apartment inspections in conjunction with building management and appropriate program staff.
