Our Community Living Department is seeking a dedicated, ASL-fluent Case Manager for our Supported Living Program, serving clients in the East Bay, particularly the Fremont area. If you're passionate about equity, access, and community integration-and you're fluent in American Sign Language -we want you on our team.

About the Role

As a Supported Living Program Case Manager, you'll oversee 6-8 Supported Living cases, including Deaf/Hard of Hearing individuals who communicate in ASL. You'll ensure that every client receives culturally and linguistically accessible services to live independently and with dignity in the community of their choice.

Key Responsibilities

  • Emergency Response: Respond to client and staffing-related emergencies (with ASL-accessible tools or back-up support as needed)

  • Client Engagement: Meet regularly with clients and their circles of support, using ASL and Deaf-accessible approaches

  • Team Leadership: Recruit, train, and support staff, including guidance on working with D/HoH clients

  • Admin Excellence: Maintain compliance with ISP timelines, training, scheduling, and documentation standards

  • Inclusion & Advocacy: Champion accessibility and representation for Deaf and Hard of Hearing individuals

What We're Looking For

  • Fluency in American Sign Language (ASL) is required for our Deaf Services Program Case Managers.

  • Experience with Deaf/Hard of Hearing clients and Deaf culture is highly preferred.

  • Education: BA in Special Education, Rehab Counseling, or related field preferred, but not required for Program Case Managers.

  • Experience: 3+ years supporting adults with disabilities (case management preferred); supervisory experience a plus

  • Skills: Excellent communication, time management, and crisis problem-solving

  • Requirements: Valid CA driver's license, clean DMV record, First Aid/CPR (or obtain within 30 days), and ability to travel within assigned counties
