Your Specific Responsibilities:
The incumbent in this position will perform intervention interviews and work independently under the immediate supervision of the Operations and Management Consultant Manager. The incumbent in this position performs HIV/STD intervention and prevention services, as per protocols, intervention interviews, partner referrals, professional client education that is aimed at reducing the risk of infection. All work is performed observing strict security and confidentiality laws regarding patient information, maintaining information in accordance with state and federal statues, rules and regulations along with Department of Health Polices, Protocols and Procedures. Duties include but are not limited to the following:
Personal management of STD/ HIV/ client case load including interviews re-interviews and interviews for infected clients and their sex partners, and close contacts. Provides HIV pre and posttest counseling both anonymously and confidentially and partner notification as indicated necessary. Also participates in the reporting process of HIV infection.
Conducts field investigations to locate and refer for examination those persons suspected to be a high risk of infection for STD/HIV. Field investigations include field interviews, locating HIV/STD patients and returning them to treatment, screening and blood tests while maintaining strict security regarding client records, client identity and the release of information about their care.
Will assist with maintaining patient flow as needed which includes obtaining pertinent past medical histories for medical charts and documentation of patient symptoms for STD/HIV clients, in the absence of designated staff this is to include STD counseling and education. Referrals to other programs are conducted based on the customer's needs.
Performs venipuncture in the field, clinic and anonymous test sites as necessary and participates in health fair screenings, always utilizing universal precautions.
Provides technical assistance and participates in a program of surveillance on matters related to STD/HIV with private physicians, hospitals, clinics and laboratory personnel in confirming laboratory results, diagnosis and treatment of patients while adhering to laws governing confidentiality.
This position will conduct monthly STARS reports, identify cluster areas within zip codes with high morbidity for intervention outreach, coordinate outreach with other programs, develop target specific flyers to bring awareness for prevention and intervention of STD’s, visit health care providers to forge partnerships, create trainings for motivational interviews with clients: Research best practices for disease intervention and prevention in other counties and state to incorporate within the STD program, review STD/HIV cases and give input for the Disease Intervention Specialist to follow up. Have proficient knowledge of/with social media networking to find partners and stay within legal confinements.
