WHAT YOU CAN EXPECT

• Diabetes Treatment Plan Development & Monitoring Assesses, monitors, and implements treatment plans for patients at various stages of the disease focusing on prevention of complications and management of stable diabetes.
• Patient Education Educates patients and families on various subjects including diabetes and the disease process; medication, diet and nutritional monitoring; foot care, prevention and care of complications; blood glucose monitoring and the techniques of self-monitoring and injection; technology, understanding new developments.
• Treatment Assessment & Recommendation Provides ongoing assessment and recommendations to the treatment team and patient/family for adjustments in the plan of care as required.
• Care Plan Development Develops and individualizes interdisciplinary nutrition care plan with short and long-term goals.
• Learning Environment Creates an educational setting conducive to learning with adequate resources to facilitate the learning process.

Qualifications

• Required Bachelor's Degree in Nursing or Dietetics/Nutrition
• Required Basic Life Support Program (BLS) - American Heart Association
• Required Registered Nurse (RN) Educator or Registered Dietitian (RD)
• Preferred 1 year Nutrition/Diabetes Care
• Preferred Certified Diabetes Educator (CDE)
