POSITION SUMMARY:

Responsible for locating and placing consumers in volunteer work experience positions in the community. Assists consumers with maximizing functional work behaviors and overcoming barriers to employment through training in appropriate work-related behaviors. Facilitates consumers’ transition from the Work Adjustment Program to the Employment Services Program and into competitive employment in the community.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Engages consumers and assesses consumers’ work readiness. Assists consumers with identifying barriers they may have to obtaining employment in the community.

• Collaborates with the consumer, the consumer’s therapist, case manager, and other team members to develop and implement an employment plan that addresses overcoming the consumer’s barriers to employment.

• Assists/coaches consumers in the development of job search skills, transportation arrangements, resume preparation, interviewing techniques and job retention.

• Teaches Peer Support Specialist courses and Work Readiness/Pre-employment workshops.

• Maintains listing of job opportunities and establishes relationships with prospective employers. 

• When appropriate visits work site, observes actual work in progress and prepares analysis of work environment, job requirements and specific job tasks.

• Meets with consumers on a weekly basis or more frequently as needed.

• Maintains ongoing liaison with supervisors obtaining feedback regarding consumer performance; assists in resolving problems quickly and efficiently.

• Accompanies consumers to placement interviews, work sites, etc. as needed.

 • Keeps program staff informed about consumer performance and support needs and follows up to assure all issues are addressed.

• Generates monthly reports reflecting consumer activity, specific problems and action taken to address the problems during that month.

• Documents in progress notes consumer activity, communication, participation, contacts and concerns.

• Attends meetings as assigned, including program employment meetings, service team meetings and staff meetings.

• Develops and maintains accurate, complete and timely documentation of services provided to consumers; ensures compliance with Pacific Clinics policies and procedures, contract compliance, state licensing and other requirements; enters information in consumer records, gathers data and may prepares reports on for management use and decision- making.

• Develops and maintains skills in the employment field through seminars, workshops or other forms of training/education.

• Interacts with other internal programs, clinical/support staff and external sources such as school or community personnel as applicable.

• Reports to work on time and maintains reliable and regular attendance.

• Communicates effectively in a culturally competent and diverse consumer population and promotes favorable interaction with managers, co-workers and others.
