Description

Job Purpose:
This is a supervisory position that is responsible to assure that people served receive services that promote the quality of life expressed through their self-direction, and in accordance with Whole Health Clinical Groups (WHCG) philosophies and management policies, as well as local, state and federal regulations, including compliance with DHS 83 licensure.

Essential Job Functions:

  1. Lead Residential Supervisor, provides clinical support, training and coaching for Residential Supervisors
  2. Supports Director and Program Manager to maximize CCS in Residential Facilities
  3. Writes and takes lead on Letters of Authority for all clients
  4. Coordinates third party petitions
  5. Provide formal teaching and support in accordance with Individual Support Plans.
  6. Complete all required documentation (Create and update Individual Support Plans, health notes, program documentation, financial recording, correspondence to families, eligibility reporting, etc.).
  7. Complete assessments and provide input into the development of services, goals, and objectives.
  8. Assure homes are clean, neat, safe, and orderly to comply with health, safety, and company standards.
  9. Provide financial management assistance to individuals enrolled in the program as assigned.
  10. Handles client daily and weekly allowances and documents disbursement of funds.
  11. Assist in arranging appropriate transportation to day programs, work, activities, and medical appointments.
  12. Assist individuals in maintaining contact with their families.
  13. Assist individuals to use their communities and its resources.
  14. Provide direct training to individuals served in order to maintain health/wellness.
  15. Monitor and document health information and concerns on a daily basis.
  16. Assist individuals to administer their medications and provide training to individuals to increase their skill in self-medication.
  17. Assist individuals to schedule, attend, participate and follow recommendations received at medical appointments.
  18. Support, assist, and discipline staff, as necessary.
  19. Develop and implement systems to supervise all employees.
  20. Develop and evaluate staff schedules to assure compliance with budgeted hours and to minimize the use of overtime.
  21. Provide specific training as assigned.
  22. Supervise payroll activities. Approve leave requested and earned by all employees supervised.
  23. Evaluate all employees supervised, with input from relevant professional staff.
  24. Understands and adheres to established policies and procedures and DHS 83 rules.
  25. Attends and participates in staff meetings, in-service training seminars and conferences as required.
  26. Participates in department quality planning activities, (e.g. continuous improvement, Logic models, performance metrics, etc.,)
  27. May have to provide group home coverage during times of direct care staff vacancy or time off.
  28. May have to provide Residential Supervisor coverage at another facility during times of Residential Coordinator vacancy or time off.

Qualifications

Required Technical Skills, Experience, Education, and Credentials:

  1. Bachelor's Degree is required or an Associates Degree in Human Services or five years of equivalent experience with working with adults with severe and persistent mental illness.
  2. Minimum of 2 years experience working with adults with severe and persistent mental illness.
  3. Minimum of 2 years supervisory experience.
  4. Must be familiar with Microsoft Word and Excel.
  5. Professional workplace appearance and conduct; friendliness and courtesy to consumers, coworkers and vendors.
