Description

THE JOB
The Integrated Health Team Director is responsible for coordinating, implementing and overseeing an array of outpatient integrated health service delivery provided by an array of staff members. You will establish a good rapport with a wide variety of agencies, organizations, and individuals. You will work closely with the treatment team personnel to facilitate admission and pre-discharge planning, care coordination, and team communication. You will be responsible to lead, coach, and foster professional growth for team members.

WHAT YOU HAVE
Required:
- Bachelor's Degree required, Master's Degree in Social Work, Counseling, or related field preferred.

- 5 or more years' experience as a professional clinician.

- 2 or more years' experience supervising personnel.

- Demonstrated knowledge of community resources.

- Experience working with persons who have or struggle with mental health challenges, substance use disorders, and/or homelessness.
