Schedule: Monday-Friday

As a Case Manager at Recovery Centers of America (RCA), you'll draw on your professional experience in case management and substance use disorder treatment to help patients build sustainable paths to recovery. In this role, you'll develop individualized aftercare plans, coordinate with clinical teams, families, and external providers, and ensure a smooth transition from inpatient treatment to community-based support. Your expertise will be critical in addressing barriers to care and empowering patients to stay engaged in their recovery journey.

What You'll Do

As a Case Manager at RCA, you will:

  • Develop and coordinate personalized aftercare plans, ensuring patients have access to necessary resources after discharge.
  • Assist patients in scheduling follow-up appointments, including primary care, substance use disorder treatment, and mental health services.
  • Advocate for patients, addressing barriers such as housing, employment, legal concerns, and social support.
  • Work closely with the clinical team to align aftercare plans with treatment goals and ensure a smooth transition to continued care.
  • Engage with families and support systems, providing education and resources to reinforce long-term recovery.
  • Maintain thorough documentation of discharge planning, referrals, and patient progress.

What We're Looking For

  • High school diploma or GED. A bachelor's degree in social work or a related field, or equivalent experience in human services, is preferred.
  • Professional experience in both case management and substance use disorder treatment, with at least one year in a behavioral health, addiction treatment, or psychiatric setting.
  • Knowledge of community resources and recovery support services, including outpatient providers, housing, employment, and legal aid.
  • Strong problem-solving skills to assess and address patient needs, barriers, and care coordination challenges.
  • Excellent communication skills to collaborate effectively with patients, families, and multidisciplinary providers.
  • Proficiency in Microsoft Office and familiarity with electronic medical records or case documentation systems.

**A former patient may not be hired at the same location where they were treated until a minimum of one year has elapsed since the completion of their treatment at that location. Former patients may be considered for employment at locations within RCA other than where they were treated within a year after treatment.
