Schedule: Monday-Friday 8:30 AM - 5:00 PM
Job Summary: The Housing Solutions Case Manager is part of the Permanent Housing Team and supports clients in the Mercy House Permanent Supportive Housing Program through all stages of service delivery. Responsibilities include conducting needs assessments, developing individualized housing plans, providing referrals and advocacy, and leading life skills workshops and community-building activities. This role emphasizes trauma-informed, client-centered care and strong documentation practices.

Essential Duties and Responsibilities:
Client Service Delivery
- Manage a caseload of 17–30 clients; engage regularly to develop and monitor individualized housing plans with measurable goals
- Use harm reduction and trauma-informed care strategies to support housing stability and prevent eviction
- Conduct apartment inspections and crisis interventions as needed
- Refer clients to appropriate community resources and services
- Facilitate social and recreational activities that promote peer support and community engagement
- Build and maintain relationships with landlords and caretakers
- Participate in on-call rotation for client emergencies
Resident Services
- Support clients in goal-setting for housing, employment, education, and life skills
- Coordinate and facilitate life skills and job readiness workshops (e.g., financial literacy, parenting, stress management)
- Organize community events, focus groups, and distribute related communications (calendars, flyers, newsletters)
- Help residents build support networks and access available services through advocacy and referrals
Outreach & Community Relations

Engage with neighbors, partners, and community service organizations to support program goals
Team Collaboration
- Partner with property management for effective communication and coordinated service delivery
- Work as a team member across departments to implement agency mission and resident programs
- Attend internal and external meetings and training opportunities
Administrative Duties
- Maintain accurate, up-to-date documentation (case notes, assessments, plans)
- Ensure timely completion of all paperwork for compliance and funding purposes
- Assist with data tracking and reporting requirements
- Learn relevant systems and services (e.g., Adult Rehabilitative Mental Health, substance use treatment)
- Perform other duties as assigned
Qualifications
Requirements:
Education & Experience
- Bachelor’s degree in Social Work or a related field preferred, with experience in mental health, substance use, or co-occurring disorders
- Candidates without a degree but with 2+ years of direct experience working with homeless, low-income, and diverse populations are encouraged to apply.\
General
- A valid CA driver’s license, proof of insurance and a reliable vehicle is required. Comfortable with utilizing your own vehicle to conduct home visits, visiting clients at shelters, meeting clients out in the community etc.
