Position Summary
Responsible for patient care under the supervision of an attending physician.
Education
Master’s or Bachelor’s degree in Physician Assistant Studies from a CAHEA accredited program
Master’s Degree in Advanced Practice Nursing Studies from a CCNEA accredited program.
Licenses and Certification
Current Illinois Licensed Physician Assistant or Advanced Practice Nurse
Current NCCPA certificate as a Physician Assistant or AACC certificate as an Advanced Practice Nurse
Physician Assistant or Advanced Practice Nurse practitioner-controlled substance licensure
Role Specific Responsibilities
Maintains collaborative relationship with supervising physician, ensuring open dialogue and adherence to feedback and recommendations for improvement.
Serves as facility lead for inpatient diabetes operations.
Dedicated consultation service for education and management of patients with diabetes.
Provides direct clinical care for patients with diabetes according to established protocols and utilizes evidence-based practice standards in collaboration with interdisciplinary team.
Supports the safe and effective use of insulin infusion protocol and transition to subcutaneous insulin regimens in critical care areas.
Promotes patient-appropriate subcutaneous insulin prescribing in the hospital.
Works collaboratively with patients to manage use of insulin pumps and other diabetes technology in the hospital.
Provides education to patients and families for self-management of acute or chronic illnesses, tailoring instructions to patients' individual circumstances. Delegates diabetes education to other members of the diabetes care team, as appropriate.
Collaborates with interdisciplinary care team for discharge planning for patients with diabetes to improve transitions from acute care to home through diabetes specific discharge recommendations/orders, including medications, diabetes technology, nutrition instructions, referrals, and continuing care instructions.
Participates in service line planning and coordination.
Acts as a liaison with other services.
Performs proper and timely billing for services rendered.
Ensures that billing and clinical documentation adhere to compliance standards.
Maintains all required records, reports, statistics, logs, files and other documents as required, including but not limited to payroll, schedules and status changes.
Compensation (Commensurate with experience):
