Join our growing Outpatient Palliative Medicine team as an Advanced Practice Provider (APP) in a hybrid role based in Palo Alto, California. This position offers an opportunity to provide compassionate, patient-centered care within a multidisciplinary team that values work-life balance and professional growth. With a mix of in-person and remote work, this role supports our mission of delivering high-quality palliative care while empowering providers with flexibility and autonomy.

Key Responsibilities:

• Deliver outpatient palliative care services, managing a caseload of 6-8 patients per day. This includes a mix of 3-4 new patient evaluations and follow-ups.
• Collaborate with a dedicated team of physicians, social workers, chaplains, and nurse coordinators to address the physical, emotional, and psychosocial needs of patients.
• Provide care independently while engaging in regular multidisciplinary discussions for support and knowledge sharing.
• Utilize a 10-hour shift structure, with patient visits typically conducted between, followed by dedicated time for documentation, education, and administrative duties.
• Contribute to quality improvement initiatives, educational programming, and team-based projects that enhance service delivery and patient outcomes.
• Participate in weekly educational sessions, with access to CME opportunities through Stanford’s platforms.
• Support efforts to improve patient outcomes in areas such as pain management, psychological support, and resource navigation.
• Schedule and Location:
• Work Schedule: Monday through Friday, 10-hour shifts.
• Hybrid Model: Combination of onsite and remote work

Team Composition:

• APPs: 5 current providers (this position would be the 6th) including one lead APP.
• Physicians: 8-9 providers with plans for further growth.
• Support Staff: 3 social workers, 1-2 chaplains, and nurse coordinators who assist with scheduling, referrals, and patient follow-ups.

Ideal Candidate:

• Preferred Experience: At least one year of outpatient palliative care experience.
• Additional Consideration: Candidates with outpatient experience in oncology, neurology, or cardiovascular care who have demonstrated an interest in palliative medicine through education or professional development.
• Inpatient Experience: Candidates with inpatient palliative care backgrounds are also encouraged to apply.
• Ability to work autonomously and manage a diverse patient population with complex needs.
