What will you be doing:
The Senior Referral Coordinator plays a pivotal role in managing the referral process to ensure patients are promptly and appropriately staffed with aligned DME and home health providers. This position acts as a central liaison among patients, healthcare providers, and internal teams, guaranteeing efficient referral processing in line with company policies, health plan requirements, and regulatory standards. Additionally, the Senior Referral Coordinator offers mentorship to junior staff, assists with complex cases, supports managers, and drives process improvements to enhance service delivery.
What will you come with:
- Strong critical thinking skills, including problem-solving, decision-making, and clinical judgment.
- Strong knowledge of DME and/or home health referral processes, provider coordination, and health plan policies.
- Ability to effectively prioritize tasks and manage multiple responsibilities in a fast-paced environment.
- Highly organized and detail-oriented with strong attention to accuracy.
- Proficiency in Windows operating systems.
- Experience with computerized medical management systems and/or web-based healthcare applications.
- Minimum of 3–5 years of experience in home health referral coordination, case management, or a related healthcare role.
- Required to uphold compliance principles as outlined in the Code of Conduct, Employee Handbook, and company policies and procedures.
