This is a fully remote position, hours are Monday-Friday 8:00 am - 4:30 pm.
Occasional weekend or holiday. Must have high speed internet.
Major Responsibilities:
Assist in the care management process
1)Independently conducts patient outreach and enrollment process including a patient/family interview and records received information into department EMR within department guidelines.
2)Research and coordinate needed services upon the direction of his/her assigned care managers
3)Conduct patient contacts over the phone or in person/in patient's home to record patient's current status, ability to follow care plan, barriers to care and alert care manager of issues/concerns within department guidelines.
4)Utilize Department EMR to document all activities and outcomes within department guidelines
5)Participate in the pre-authorization process under direction of the care manager; reviews/triages care managers physician practice EMR task list to ensure questions, referrals, issues handled or forwarded to the assigned care manager.
6)Works independently and upon direction from care management staff to file, prepare correspondence, data entry, system documentation and other medical management related activities.
7)Reviews, retrieves tasks/messages for assigned CM within the APP/AMG physician practice EMR.
Effectively and Efficiently communicates with patients and healthcare professionals to ensure smooth care transitions.
1)Follows communication and documentation department standards
2)Communicates effectively and efficiently with multiple providers and resources concerning a patient’s healthcare needs using multiple modalities; email, phone, etc.
3)Keeps care management team members apprised out outcomes of assigned tasks in a timely and effective manner to promote safe outcomes.
4)Identifies and maintains appropriate communication with supervisor involving problems and observations in course of daily operations.
5)Communication with case managers, managers, directors, other health care professionals, and various staff in a positive fashion in order to promote behaviors of excellence, patient satisfaction, quality services delivered and resolution of issues.
6)Effectively uses patient engagement strategies to reach and enroll patients, assist patient’s in self-actualization in their healthcare needs.
7)Manages workflow tasks according to Department standards within the CM software
Education/Experience Required:
AA degree or equivalent and 3-4 years experience working in a managed care organization, hospital or physicians office.
Knowledge, Skills & Abilities Required:
Good verbal and written communication skills. Ability to work as a team member. Competent computer keyboard, laptop manuvering, competent in Windows, Excel, database entry. Good organizational skills. Ability to handle difficult calls and maintain professional conduct. Willingness to be flexible with job responsibilities
Competent Medical terminology ability/experience
Physical Requirements and Working Conditions:
Ability to work under stressful conditions. Ability to communicate denial of services to members and providers. Excellent phone mannerisms, ability to effectively engage conversation, motivate and engage patients to participate in our program, and speak to all types of patients, particularly seniors. Regional travel required for meetings and patient home visits May need to flex daytime hours to stretch to 8pm M-F , and Saturday mornings to reach pts after work hours.

Pay Range
