About the role

<p><strong>Position Objective:</strong></p><p>Under the direct supervision of a Maryland-licensed pharmacist and in coordination with other technicians, the Pharmacy Technician-Medication Reconciliation (Med Rec) performs a comprehensive admission medication history for patients in predetermined focus areas of patient intake.&nbsp; An accurate medication list is created that enables effective medication reconciliation to occur.&nbsp; This job responsibility requires knowledge of commercially manufactured drug products available on the retail market, meticulous attention to detail, excellent communication ability, and strong customer service skills.</p><p><strong>Essential Job Duties:</strong></p><p>Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.</p><ol><li>Demonstrates in-depth knowledge of drug generic-trade names, dosage forms and strengths marketed, and exhibits functional familiarity with herbals and over-the-counter (OTC) medications.</li><li>Uses appropriate pharmaceutical-medical terms, abbreviations, and symbols common to pharmacy practice.</li><li>Performs calculations required for the usual dosage determinations and course of therapy duration.</li><li>Demonstrates proper telephone etiquette during phone calls conducted as part of the medication history validation process, and follows up appropriately to requests for assistance.</li><li>Interacts effectively and professionally with patients and their families, as well as with Pharmacy colleagues and other members of the health care team.&nbsp; Is always in full compliance with the hospital dress code policy.</li><li>Maintains confidentiality of patient information.</li><li>Participates in the orientation and training of new Med Rec staff when requested. Provides clear and accurate instructions, demonstrates the required scope of activities, and reviews the new trainee’s work for accuracy following program guidelines.</li><li>Meets program standards for accuracy and volume, contributing to meeting metric goals for this key Pharmacy Department initiative.&nbsp; Meticulously follows program methodology for researching background information to validate patient personal reporting, e.g. Surescripts, retail pharmacies, or skilled nursing facilities.</li><li>Accepts instruction and coaching for improvement of skills in an open and professional manner, incorporating corrective improvements into daily practice without constant supervision or repeated reminders.</li><li>Collaborates in a positive manner with ER staff personnel, and complies with posted cautions for use of personal protective equipment or restricted access to patients.</li></ol><p><strong>Educational/Experience Requirements:</strong>
