Clinical Pharmacy Specialist - PRN - AAMC

<p>&nbsp;</p><p><strong>Position Objective:</strong></p><p>Responsible for carrying out Clinical Pharmacy services through promoting evidence-based pharmacotherapy and sound pharmaceutical care through the development of medication management protocols, and drug therapy policies. Serves as a Clinical Pharmacy resource for the health system.</p><p><strong>Essential Job Duties:</strong></p><p>Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.</p><ol><li>Conducts departmental activities to meet the goals, objectives and policies of the pharmacy department.</li><li>Participates in improvements in interdepartmental policies and procedures, which support the Pharmacy Department and Hospitals strategic goals. Maintains accurate records as required by departmental policies and procedures and/or federal and state board regulations.&nbsp;</li><li>Completes all core competencies and documents clinical activities accurately and according to policies and procedures. Supports the Investigational Drug Service in the daily operations of the department.</li><li>Serves as the medication expert on medical and surgical units and is available to provide time-sensitive, accurate information for other healthcare providers in acute scenarios.</li><li>Provides pharmaceutical care, dosing and guideline recommendations to impact patient care and decrease readmissions. Provides guidance for drug selection and pharmacotherapy regimens in accordance with evidence-based guidelines.</li><li>Completes pharmacotherapy drug class reviews, formulary addition/deletion, assimilation of evidence-based protocol/order-sets, medication safety related activities and/or targeted medication expense reductions.</li><li>Assists in appropriate transition of care services, including medication reconciliation and discharge education of high-risk patients (or those with pertinent medication-related problems) as appropriate.</li><li>Serves as the pharmacy representative on multidisciplinary committees and develops pharmacotherapy-specific specific educational programs, protocols, standard operating procedures, and order sets.</li><li>Educates other healthcare providers, via formal and informal in-services, on approved treatment guidelines and criteria to increase appropriate and cost-effective use of medications in the medical and surgical units.</li><li>Serves as a resource for Pharmacy School Students and AAMC Pharmacy Residents.</li></ol><p><strong>Educational/Experience Requirements:</strong></p><p>The minimum level of education and experience for this position includes:</p><ul><li>Doctor of Pharmacy degree required</li><li>Completion of PGY-1 residency
