Job DetailsJob Location: Monroe Georgia - Monroe, GA 30655Position Type: Full TimeProvidence Care is seeking an experienced Central Intake Manager (RN) to lead our centralized hospice intake team. This role ensures timely, compliant, and clinically appropriate admissions while supporting strong communication between referral sources, clinical teams, and leadership.
The ideal candidate is a Registered Nurse with hospice or home health experience who brings strong clinical judgment, operational leadership, and a passion for improving patient access to hospice care.
This position plays a key role in ensuring referrals are processed efficiently, eligibility is appropriately assessed, and patients receive the care they need as quickly as possible.
Key Responsibilities
Clinical Review & Eligibility
Review incoming hospice referrals to determine eligibility based on Medicare and regulatory guidelines
Collaborate with the Medical Director and clinical leadership on complex eligibility decisions
Ensure documentation supports terminal diagnosis and regulatory requirements
Identify and escalate potential compliance risks
Intake Operations
Oversee the daily operations of the centralized intake department
Ensure referrals are processed accurately and in a timely manner
Monitor intake performance metrics such as response time, referral conversion, and documentation completion
Coordinate with branch clinical teams to ensure smooth transitions from referral to admission
Leadership & Team Management
Supervise and support intake staff, including clinical and non-clinical team members
Provide training on hospice eligibility, documentation standards, and intake workflows
Conduct performance reviews and support staff development
Promote a culture of accountability, responsiveness, and teamwork
Collaboration & Communication
Serve as a key liaison between referral sources, clinical teams, and leadership
Support business development teams by ensuring timely referral follow-up
Maintain professional communication with hospitals, physicians, and community partners
Quality & Compliance
Ensure intake processes comply with CMS regulations, state requirements, and HIPAA standards
Maintain audit-ready documentation and support regulatory readiness
Identify opportunities to improve intake processes and patient access
Qualifications
Active Registered Nurse (RN) license
3+ years of clinical nursing experience
Hospice or home health experience
Strong understanding of hospice eligibility and Medicare guidelines
Experience reviewing clinical documentation
Strong organizational and communication skills
Preferred
Leadership or supervisory experience
Experience working with EMR systems
Experience in hospice intake or admissions
