Nurses at Uphams PACE are core members of an Interdisciplinary Care Team responsible for patient assessments as well as contributing to a patients plan of care. Other important responsibilities include providing nursing care and prescribed treatments to participants in their homes or at a PACE center. Call us to learn more.
Primary Function:
Provide primary nursing services to participants at the Adult Day Health Center. Provide initial and semi-annual nursing assessments; develop and implement nursing plan of care; provide medication management, daily nursing treatments, episodic assessments and nursing interventions at the PACE center. Collaborates with Primary Care to provide complete nursing services in a safe, secure, accessible environment with skill, dignity and respect.
Qualifications
Valid MA RN license
