Job Summary

Responsible for coordinating outreach to pediatric practices and key community partners, developing and updating communication tools to accompany outreach activities, working with the Pediatric Support System team to develop a practice feedback response mechanism, managing referrals through EPIC according to system protocols, establishing appropriate system metrics in order to collect and analyze data, and coordinating a Project Advisory Board. Key activities include referral management, practice outreach, data collection and analysis, system integration and capacity building.

Essential Functions

  • As a member of the Pediatric Support Services (PSS) team, contributes to systems building, promotes model fidelity and assists in the development of PSS procedures and protocols.

  • Responsible for referral assessment/triage, referral management and outcomes documentation for referrals made to the PSS through EPIC.  Monitors system response and referral connection rates.  Documents referral trends and identifies gaps and barriers to connection. 

  • Conducts outreach to all Prisma Health pediatric practices and key system partners.  Develops academic detailing protocols and communication tools that are utilized in outreach activities.  Reviews the practice feedback response mechanism in coordination with the PSS team.  Implements updates and improvements to outreach efforts as directed. 

  • Serves as the PSS system liaison to the Pediatric Behavioral/CONNECT team providing guidance in system building and fidelity. 

  • Assists with community outreach, resource identification and database management.

  • Collects PSS data according to established system metrics in a Redcap template.  Contributes to data analysis efforts to improve system response, connection rates and care coordination efforts. 

  • Coordinates the PSS Project Advisory Board and serves as the liaison between the Board, the project team and the Bradshaw Institute for Community Child Health & Advocacy.

  • Performs other duties as assigned.

Supervisory/Management Responsibilities

  • This is a non-management job that will report to a supervisor, manager, director or executive.

Minimum Requirements

  • Education - Bachelor's degree in Social Work, Nursing or Psychology. Master's degree in related field of study preferred.

  • Experience - Two (2) years of related experience. Pediatric direct care experience is preferred. Experience in Epic, data collection and analysis experience preferred
