This position provides telephone assessment and referral services based on required guidelines and HealthChoices standards to help people in our county find the best programs and services to meet their individual and family needs. Examples of duties

This position assesses consumer needs by telephone, explains key behavioral health care information, provides authorization and appropriate referrals, and participates in team support and development activities. As a Clinical Care Reviewer (CCMM), you will perform the following duties:


  • Conduct Telephonic Assessments: Gather demographic and clinical information necessary to make emergency, urgent, and routine referrals for mental health and substance abuse services
  • Share Information: Provide consumers with information regarding benefits, treatment options, the managed care system, and community resources
  • Determine Care Needs: Review and evaluate the medical appropriateness of psychiatric and behavioral health and substance abuse cases and, in consultation with supervisor, render a certification decision
  • Serve as an Advocate: Support and promote the interests of consumers and families; facilitate coordination of care with other care managers; and conduct follow-up with consumers and service providers 
  • Ensure Quality and Credentials: Participate in customer service assessment activities to assure quality; participate in clinical rounds, seminars, and training sessions to maintain licensure and clinical credentials
  • Complete Documentation: Maintain accurate data, referrals, and case documentation in the automated system and client records, in accordance with program standards; prepare required reports as mandated by federal, state or county policies and procedures


Work Schedule and Additional Information:

  • Full-time employment
  • Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with a 30-minute lunch.
  • Telework: You will not have the option to telework in this position.

Qualifications

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • Two years of professional mental health clinical experience and possession of an active license to practice as a Registered Nurse issued by the Pennsylvania State Board of Nursing; or
  • A master's degree in social work, psychology, nursing, or counseling and two years of professional mental health clinical experience.
