Description
We are seeking a compassionate and experienced PRN Home Health Registered Nurse (RN) to join our dedicated team. In this role, you will provide comprehensive nursing care to patients in their homes. This position allows for flexible scheduling, making it ideal for those who prefer working on an as-needed basis.

Responsibilities:
· Plans, coordinates, implements, and evaluates the nursing care of all patients.
· Utilizes clinical knowledge and skills to identify patient needs and coordinate services and referrals to meet these needs
· Works closely with the inter disciplinary team to provide a continuous appropriate, comprehensive, and responsive plan of care for patients.
Requirements
· A valid non-restricted RN license in the state of Colorado is required.
· Must have a valid non-restricted driver’s license.
· Flexibility and availability to work a weekend schedule if needed.
· At least 1 year of home health experience preferred.
