Practice or research specialty/focus of the position: Heart Services – Cardiac Intensive Care Unit

 

Position Summary:   This clinical pharmacist position plays a key role in the Clinical and Patient Care Services Division.  While this position has many challenging responsibilities, the major areas are listed below:

• Provides in-depth clinical pharmacy services to Heart Services patients in the Cardiac Intensive Care Unit (CICU)

1. Primary Order verification for 7 east (CICU) patients 0700- 1530; dayshift Monday- Friday
   • • Adhere to policies / restrictions around medications and obtain second level approval or other approvals as required by hospital restrictions
     • Assist with other Heart Services areas order verification as needed
   • Respond to emergency events on CICU (Code Blue, RRT, code stroke events) and facilitate urgent/ emergent medication needs for patients in CICU     
   • Document completed Admission Medication Histories notes in the medical record per department procedures
   • Monitor and document clinical monitoring activities per hospital policies and procedures for medications such as Vancomycin, Aminoglycosides, Antiarrhythmics (Sotalol & Dofetilide), and therapeutic anticoagulation.
     • Document monitoring notes in the medical record per hospital policy
   • Document for required Medication Risk Evaluation and Mitigation Strategy (REMS) programs per hospital/ health-system policy in the EPIC system
   • Adhere to hospital policies and procedures related to patient home medication storage and patient take home medications while admitted in the hospital
   • Verbal Handoff to Pharmacy team member covering CICU on evenings and receive handoff from night shift for clinical needs on other shifts
   • Perform IV to PO, Antibiotic dose adjustment protocol for renal function and review other high cost or high risk medications for cost savings or safety improvements.
   • Document interventions in the EPIC system
   • Document SRS reports in the RL6 system as appropriate
   • Serve as pharmacy support resource for medication shortage management issues
   • Serve as a pharmacist preceptor for PGY1, PGY2 residents and student learners on rotation

• Maintains outstanding skills as a clinical pharmacy practitioner and consistently demonstrates the value of pharmacy services
• Educates medical, nursing, and pharmacy staff about the management, prescribing and monitoring of therapies used in patient care area(s) of responsibility.
• Serves as a resource to the Department of Pharmacy for area(s) of expertise.
• Participates in departmental and hospital committees and initiatives related to practice area.
• Contributes to student and resident education
• Provides educational lectures and/or participates in research (Duke, State/Local, National)
• Works with peers to achieve division and departmental goals
• Takes responsibility for personal and professional growth
• Maintains familiarity with all departmental programs and works as integral part of the pharmacy team.

 

Additional Clinical Work Responsibilities

1. Rotating 1 Clinical Staffing weekend per month
2. Rotating Night Shift (2-3 nights/ year)
3. Night Shift backup on call coverage (Approximately 1 week/year)
4. 1 Major Winter Holiday Block Coverage (Thanksgiving, Christmas or New Year’s and 1 Summer Holiday (Memorial Day, 4th of July or Labor Day)
