What You’ll Do
- Deliver personalized, high-quality care to older adults in homes and senior living settings.
- Conduct comprehensive assessments and develop individualized care plans.
- Manage chronic conditions with skill and empathy.
- Collaborate across clinical disciplines (primary, psych, palliative, neuro) for whole-person care.
- Build trusted relationships with residents, caregivers, and community partners.
- Maintain thorough documentation and consistent communication.
What You’ll Bring
- Active, unencumbered NP or PA license in the state you will be practicing
- Minimum of 2 years of clinical experience (geriatrics or primary care preferred)
- Compassionate, patient-centered approach and strong clinical judgment
- Valid driver’s license and reliable transportation
