Summary:

The Social Worker is a key member of the interdisciplinary team who provides and oversees the provision of psychosocial care for selected patients and families. Conducts bio-psychosocial assessments, provides intervention and treatment as indicated. Identifies high‑risk psychosocial factors of patients/families that impact health status and helps staff understand the influence of those factors upon the course of medical care. Collaborates with and provides social work consultation to other disciplines within the healthcare setting and the community.

Responsibilities:

Works with patients/families to navigate through the healthcare system (post-acute choices, insurance plans, etc.) to coordinate.Participates in discharge planning including coordinating patient transfers to other facilities and coordination of community.Organizes and facilities care conferences with patients, families, representatives, and other members of the care team.Collaborates with the interdisciplinary team and consults with nursing staff to regularly evaluate the patient's current and ongoing needs.Collaborates closely with governmental agencies (such as DCF, APS, CMS, and KePro) to ensure safe post-acute care.Assesses and initiates crisis interventions and baker act if necessary.

Required to identify and intervene for suspected abuse including sensitivity to age specific concerns; physical/emotional/sexual abuse of children and adolescents; domestic abuse for adults; and caretaker/institutional abuse for senior adults. Must be able to manage crisis situations, demonstrating ability to quickly appraise the crisis situation and select appropriate intervention. Able to document direct service in patient’s medical record after each significant encounter. Must demonstrate proficiency in use of the electronic medical record. Must be able to demonstrate good problem-solving skills, root-cause analysis, and principles of clinical performance improvement. Required Work Experience: One (1) year social work experience in a health care setting.
