This role partners closely with the Program Specialist to ensure daily operations, staff support, documentation, and service coordination reflect compassion, accountability, and an unwavering belief in human potential.
Overview / General Description
The Team Leader – SE/UCPS provides direct support to individuals enrolled in Supported Employment (SE) and Unlicensed Community Participation Services (UCPS) programs while assisting the Program Specialist with day-to-day operational coordination. The role supports staff coaching, service documentation, scheduling coordination, regulatory compliance activities, and communication across programs and community settings. This position serves as a functional leader and resource to staff working in community-based environments and helps ensure continuity, quality, and person-centered service delivery. The Team Leader also supports program documentation processes, satisfaction surveys, service activity tracking, and participation in interdisciplinary meetings to help individuals achieve meaningful outcomes.
Essential Functions / Key Responsibilities
• Support and monitor SE and UCPS staff working in community settings by providing guidance, coaching, communication, and problem-solving support to promote consistent, person-centered service delivery.*
• Provide direct support services to individuals participating in SE and UCPS programs, including community-based support, engagement, transportation assistance, and documentation to ensure individualized goals and service needs are met.*
• Maintain accurate and timely documentation, individual records, service activity logs (SALs), and program information within approved systems, including CareLogic, to support compliance, continuity of services, and regulatory requirements.*
• Assist the Program Specialist with operational coordination activities, including tracking required staff Learning and Development completion, driver documentation, satisfaction surveys, and other program-related administrative processes.*
• Participate in interdisciplinary meetings, including Individual Support Plan (ISP) meetings, Supports Intensity Scale (SIS) assessments, intake meetings, and team collaboration activities to support coordinated services and individualizes outcomes.*

• Maintain collaborative working relationships with internal departments and community partners to support coordinated, responsive services for individuals and families.
• Monitor operational trends, local challenges, and regulatory updates that impact the program; communicate relevant observations to leadership and support continuous improvement efforts.
