Who We Need
The ACT Independent Living Specialist (ILS) is part of the Assertive Community Treatment (ACT) Team, providing direct treatment, rehabilitation, and support services to consumers in a community setting. Responsibilities include assessing consumer needs, developing individual service plans, advocating for consumers, liaising with families and agencies, and providing crisis intervention, including rotating on-call, weekend, and holiday coverage.
MINIMUM QUALIFICATIONS:

High School diploma/GED and four plus (4+) years direct care experience, or;
Any combination of direct Behavioral Health experience/education totaling five (5) years of experience is acceptable. Example: Bachelor’s degree with one (1) year of experience.
One (1) year education and one (1) year professional experience in, substance abuse treatment and recovery.
Requires a call rotation, this may include weekend / holiday work.
Requires the use of your personal vehicle.
Ability to meet Copa’s driver authorized driving requirements, set forth by Copa Health’s insurance company. Including AZ Driver’s license upon hire, 39-month MVR, proof of insurance and vehicle registration, if applicable.
Must have demonstrated experience in Microsoft Office applications.
Satisfactory work records as determined by the People 1st
Successfully completes a pre-employment drug screening.
Complete all on-boarding paperwork, in a timely manner and be cleared to attend New Employee Orientation (NEO), as scheduled.
Ability to obtain and maintain a Level One Fingerprint Clearance Card and any other required background checks.
Bi-Lingual, preferred.

JOB SUMMARY (Including, but not limited to)

Works as part of a clinical team to facilitate recovery and inspire hope for consumers while supporting their achievement of service plan goals. Provides ongoing assessment and feedback as to their progress in relation to their service plan, regardless of the consumer’s current location.
Meets with the consumers in the community in accordance with established policies and procedures and clinical protocols to provide supportive counseling skill development and other support services as needed.
Works with consumer’s Prescribing Clinician, Nurse, and other members of the clinical team to ensure the timely development and implementation of the individual’s service plan (including all necessary assessments) and their crisis plan.
