About the role

Director Care At Home Services leads the development and execution of a Health delivery system within COPE Health Solutions and CHS Medical Group, focusing on improving the health outcomes of a specific group of individuals. Develop and implement a Care delivery strategy that aligns with the organization's goals and objectives.
Use data analytics to identify trends, patterns, and opportunities for improvement within the population.
Design and implement targeted interventions and programs to address specific health needs and improve outcomes.
Collaborate with various stakeholders, including physicians, nurses, care managers, and payers, to ensure effective implementation of the strategy.
Monitor the performance of the population health strategy and makes adjustments as needed.
Work to improve the quality and cost of care for the population.
Engage with external stakeholders, such as community organizations and regulatory agencies, to promote the care delivery strategy and health outcomes.
Mentor and develop talent within the organization to support the population health strategy.
Understands and manages the financial implications of the population health strategy.
Ensures compliance with relevant regulations and policies.
Develop clinical and operational processes for the CHS
