About the Department
The Corewell Health West Ambulatory Care Management Department is made up of Social Work Behavioral Health Specialists, Social Work Psychotherapists, RN Care Managers, Community Health Workers, and Medical Assistants. Our team is accessible at various service points across our system including primary care locations, specialty clinic spaces, virtually/remotely, and in school clinic settings.
Overall, the Ambulatory Care Management department is focused on identifying and connecting with patients who have behavioral health concerns or chronic medical conditions and assist these patients with developing healthy behaviors and coping skills that will improve health outcomes. Team members of this department play an active role in the patient’s care and engage as part of the interdisciplinary team in developing an appropriate treatment approach for patients experiencing chronic disease and/or behavioral health concerns to promote self-management and symptom improvement.
The Ambulatory Care Management Department has multiple unique approaches in our work. First, our Transitions of Care Team provides comprehensive transition of care support to patients recently discharged from the hospital to help keep these patients well in the community, prevent readmissions, and reduce overall cost of care. Next, our RN Care Managers and Social Work Behavioral Health Specialists engage with patients at outpatient primary and specialty care spaces to help address chronic medical conditions and behavioral health concerns. Supportive interventions focused on reducing barriers related to Social Determinants of Health (SDoH) are provided to primary care patients by our remote team of Community Health Workers. Finally, our remote School Based Social Work Psychotherapists provide outpatient behavioral health services virtually to local high school students.
The Ambulatory Care Management Department has consistently demonstrated successful outcomes through the interventions provided to patients.
Scope of work
· Assess and treat patients referred by school staff, parents, PCP offices, and self-referred patients.
· Provide individualized therapy services and monitor patient progress.
· Develop individual plan of care and give recommendations to school staff, parents, PCPs, specialists, and other members of the interdisciplinary care team regarding mental health treatment strategies and identifies approaches to maximize continuity of care across the continuum.
· Respond to urgent mental health concerns, including suicide risk assessments and assist in connecting patient to appropriate level of care based on clinical assessment.
· Assist with greater Ambulatory Care Behavioral Health teams work as assigned.
· Participate in professional development opportunities through peer case review and monthly continuing education opportunities.

Qualifications
- MSW Required
- 2 years of relevant experience related field Required
- Licensed Master Social Worker (MSW-Master) - State of Michigan Upon Hire required.
