How you will make an impact: 

Vista Prairie at Goldfinch Estates is seeking Community Care Coordinators to join our compassionate team. As a dedicated Community Care Coordinator, you will embrace a resident-centered approach, treating every individual with unwavering dignity, regardless of their disease, diagnosis, or progression. While providing a therapeutic environment that maintains each resident’s highest level of physical, social, and physiological well-being, you will find ways to bring smiles to the faces of those you serve. You will also answer resident emergency calls promptly, assist in transporting residents to/from appointments, activities, meals, and social programs as necessary. As a Community Care Coordinator, you will also assist residents in preparing for activity and social programs as necessary and much more! Together, we can make a meaningful impact on the lives of our residents.

Schedules Vary: 

• Evening Shifts (2:00 PM to 10:00 PM); or
• Overnight Shifts (12:00 PM to 8:00 PM)
• Shifts vary depending on availability at time of interview

What you will need: 

• High school diploma or GED required.

• Must be 18 years of age or older.
• Healthcare experience required.
• 1 year of supervision experience preferred.
• Experience with leadership and guiding teams is preferred.
• Demonstrated ability to read, write, and carry out directions required.
