About the role

Memorial Community Hospital & Health System (MCH) is seeking a compassionate and highly organized RN Primary Care Navigator to join our team. This role is central to delivering a proactive, patient-centered care coordination model that improves access, enhances patient outcomes, and supports individuals in navigating the healthcare system.

As a Primary Care Navigator, you will serve as a consistent point of contact for patients, guiding them through their care plans, connecting them to resources, and collaborating closely with providers and care teams to ensure seamless, high-quality care.

This position is made possible through recently awarded grant funding that supports MCH’s continued investment in innovative, patient-centered care. Employment in this role is contingent upon ongoing funding and program needs.

What You’ll Do:

Shift Details:
• Monday-Friday 8:00a-4:30p, 40 hours per week.

Care Coordination & Patient Support
• Collaborate with patients and providers to deliver efficient, high-quality, and cost-effective care
• Develop, implement, and monitor individualized care plans using evidence-based guidelines
• Track patient progress and intervene early to support improved outcomes

Access & Continuity of Care
• Coordinate scheduling for primary and specialty care services
• Conduct post-discharge follow-up after hospital or emergency visits
• Manage referrals, authorizations, and required documentation

Team Collaboration & Communication
• Serve as a central liaison between patients, providers, and multidisciplinary teams
• Prevent gaps, duplication, and conflicts in care delivery
• Build strong relationships with internal teams and community partners

Patient Advocacy & Education
• Guide patients through the healthcare system as a single point of contact
• Provide health education, coaching, and chronic disease support
• Advocate for patient needs with empathy and professionalism

Social Determinants of Health (SDOH)
• Screen for social and economic barriers to care
• Connect patients to resources such as housing, food, transportation, and financial assistance
• Integrate social support into patient care plans

Medication Management
• Maintain accurate medication lists and perform reconciliation
• Coordinate prior authorizations and assistance programs
• Educate patients on proper medication use and adherence

Quality & Compliance
• Document all interactions and care coordination activities in the EHR
• Ensure compliance with MCH policies, HIPAA, and Rural Health Clinic regulations
• Support quality improvement and performance initiatives

What You’ll Need:

Education & Licensure
• Graduate of an accredited School of Nursing
• BSN strongly preferred
• Current RN licensure in Nebraska, Iowa, or compact state
• Current BLS certification

Experience
• Minimum 2 years of RN experience in a clinic or provider practice preferred
• Experience in care coordination, case management, home health, or behavioral health preferred
• Experience navigating community resources and healthcare systems strongly preferred

Skills & Competencies
• Strong knowledge of care coordination, chronic disease management, and preventive care
• Understanding of social determinants of health and insurance processes
• Excellent communication, collaboration, and patient education skills
• Strong critical thinking, organization, and time-management abilities
• Ability to manage multiple priorities in a fast-paced environment
• Proficiency in EHR systems and healthcare technology
• Commitment to confidentiality, quality improvement, and patient-centered care
