About the role
The Assessor Reviewer II is responsible for face-to-face, in-person assessments of vulnerable populations in the District of Columbia. Persons being assessed reside in their homes, the community, or in facilities. The incumbent may also complete review decisions regarding services needed for the person(s) to be successful in the community. This position may also serve as a mentor or preceptor for new staff, assisting in the formal training of assessors, completing quality assurance competency reviews, and may be involved in process improvement initiatives.
This position will begin on December 30th, 2024. While the position will be primarily field-based, there will be required on-site meetings, pre-scheduled and as needed.

Essential Functions
- Performs face-to-face assessments of DC residents.
- Documents assessment results for the individual member status. Negotiates service plan as needed.
- Completes quality assurance competency reviews and other tasks/projects as assigned such as new assessor training.
- Provides ongoing education.
- May communicate with all health care providers to ensure appropriateness of care.
Requirements
- Four-year degree or master’s degree in nursing and/or social work.
- Minimum of three years of experience.
- Registered Nurse (RN) or Licensed Independent Clinical Social Worker (LICSW) with a current and unrestricted District of Columbia license.
- Case management and Medicaid experience preferred. Experience completing level-of-need assessments preferred.
