San Diego’s St. Paul's PACE program (Program of All-Inclusive Care for the Elderly). This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare teams at St. Paul’s PACE provide a hands-on approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone.
This is a temporary position with an anticipated duration of 6 months. The typical schedule is Monday - Friday 8:00 am to 4:30 pm.
Job Summary: The Medical Records Coordinator II maintains the participant electronic medical record system. The Coordinator handles release of information and medical record requests for prospective and active participants. The Coordinator also compiles Interdisciplinary Assessment and Treatment summaries, supports HIPAA and Medicare Part D compliance oversight, and provides assistance to QA as needed. This role will assists Center Directors/Supervisors with auditing to ensure timely completion of Assessment and Care Plans. This position fall under the direction of the Health Plan Manager while providing day to day support of the Center Director and Interdisciplinary Team (IDT).
Qualifications and Requirements:
• Education: High school diploma or equivalent.
• Experience: One year of experience in Medical Records or another clerical setting in the long-term care environment..
Duties & Responsibilities:
• Attend daily morning meeting and run PCO to locate and display all records needed for IDT review of all participants discussed.
• Track HPMS reportable data discussed at IDT and provide to QA Manager and/or Health Plan Manager.
• Handle medical records requests from Enrollment Department for new clients.
• Obtain copies of medical records requested by clinic and QA from outside facilities.
• Work with Rehabilitation Manager to prepare authorization letters for SNF to reflect skilled service/RUG rates.
• Ensure Medicare Part D compliance by tracking hospitalizations and acute SNF stays to report Medicare Part A stays. Send notification to pharmacy and pharmacy benefit manager at both the beginning and end of the Part A stay.
• Supports initiatives of the Health Plan Department related to the Electronic Health Record system.
• Assist QA Manager with handling grievances and appeals as needed.
