Senior Doc is expanding in the Los Angeles, CA area [Boyle Heights, West Hollywood, Hollywood, Alhambra] , and we're on the lookout for a Certified Nurse Practitioner or Physician Assistant with a passion for serving the senior population.

What You'll Do:

• Provide in-home primary care visits for seniors in private homes, assisted living, and skilled nursing facilities.

• Complete comprehensive assessments, diagnose and manage chronic conditions, and develop personalized care plans.

• Collaborate with care teams to deliver holistic, patient-centered care.

• Build trust and rapport with patients and their families through strong communication and a warm, compassionate approach.

Qualifications:

• Active and unrestricted NP or PA license in California.

• Experience in geriatrics, primary care, or home-based care preferred.

• A heart for seniors and a drive to provide accessible, relationship-based care.
