Position Overview:

Develops and operationalizes health system procedures to optimize the capabilities of patient logistics and capacity management under the direction of the COO. Works collaboratively with all health system leaders to design, implement, and monitor strategies to ensure adequate patient access, logistics and throughput.

MINIMUM REQUIREMENTS

Education: Registered Nurse in New Jersey. BSN. Licensed Nurse Practitioner preferred.

Experience: Five years of experience in a health system leadership position.

Other Credentials: Registered Nurse - NJ

Knowledge and Skills: Strong data analysis skills. Previous bed management system experience. Intermediate report-building experience.

Special Training: Experience with design process. Knowledge of patient logistics and possesses ability to increase patient discharge efficiency.

Mental, Behavioral and Emotional Abilities: Excellent organizational skills. Works well under pressure. Meets goal deadlines.

ESSENTIAL FUNCTIONS

  • Serves as a transformational leader accountable to optimize patient-centered flow and logistics across the Health System, community, and region through strategic planning, operational execution, and relationship management.
  • Leads strategy and execution of the health system Escort and Patient Logistics departments. Provides oversight for Capacity Command Center which includes logistics and transfer center, UR/CM/CDI/population health, clinical staff, reimbursement, and LOS.
  • Oversees the successful completion of all health system capacity management initiatives and activities. Assigns project work and assesses resource allocation for achievement of health system capacity management goals.
  • Directs the health system integrated data-driven and standardized strategic approach to foster the timely and efficient movement of patients throughout the health system.
  • Creates a health system wide Physician Advisor Program to perform real time peer reviews and increased utilization. Creates and sustains a process to analyze discharge progression, discharge delays, and barriers between health system departments that impact patient flow and access.
  • Incorporates the latest in system engineering, predictive analytics, and innovative problem solving to better manage patient throughput. Focuses on patient-centered flow that ensures the best outcome of patient care using all available health system resources.
  • Engages, partners with and leads teams across the health system that are accountable for patient flow, discharge planning, utilization management and enable effective coordination and communication between stakeholders to develop care transition plans and facilitate patient placement.
  • Provides strategic direction for the development, management, and ongoing operations of the health system-wide bed management, patient flow, and transfer teams.
  • Collaborates with clinical teams, revenue cycle, Logistics and Transfer Center, CHMG, and other departments across the health system. Collaborates closely with staffing office to ensure adequate staff are available to meet capacity demands across the health system.
  • Develops and implements policies and procedures that address health system patient throughput functions. Facilitates and monitors the flow of patients throughout the health system to maximize patient throughput with a focus on bed management across all services including demand/capacity decisions.
  • Establishes, implements, and communicates the strategic direction of the health system patient logistics and throughput operations division. Identifies, recommends, and implements new processes, technologies, and systems to improve and streamline health system patient flow and throughput processes.
  • Establishes, communicates, and implements health system patient throughput operations-related policies, practices, standards, and security measures to ensure effective and consistent support and execution. Establishes and administers the department’s budget.
  • Performs other duties as assigned.
