About the role

Sturdy Health is seeking a passionate and dedicated Community Health Navigator to support our Maternal Equity Program funded by the CHSF Grant. This role focuses on improving health access and equity in maternal care by addressing barriers to care and connecting expectant and new mothers with essential resources. The Navigator will work closely with at-risk birthing populations, ensuring culturally competent care and fostering a supportive network to improve maternal and neonatal outcomes.

Education/Training

  • Experience in community health, patient advocacy, or maternal health programs.
  • Strong understanding of social determinants of health and cultural competence.
  • Excellent interpersonal and communication skills.
  • Bachelor’s degree in public health, Social Work, Nursing, or a related field preferred.

Licenses/Certification:

  • Certification as a Community Health Worker or equivalent is preferred.
  • Valid driver’s license and access to reliable transportation required.

Required Qualifications and Skills:

  • Minimum of 2 years of experience in community health, social work, or a maternal health-focused role.
  • Deep understanding of health equity principles and social determinants of health, particularly as they relate to maternal and neonatal care.
  • Demonstrated ability to work effectively with diverse populations, including economically, racially, and linguistically diverse groups.
  • Experience conducting outreach and navigating patients through complex healthcare systems.
  • Strong organizational and time-management skills with the ability to prioritize tasks.
  • Proficiency with electronic medical record (EMR) systems and data reporting.
  • Excellent verbal and written communication skills.

Preferred Qualifications and Skills:

  • Experience in developing and delivering culturally sensitive educational materials.
  • Familiarity with maternal health issues, including gestational diabetes and prenatal care coordination.
  • Multilingual proficiency, especially in Haitian Creole or Spanish.

Essential Job Functions:

  • Patient Advocacy and Support
    • Assist expectant and new mothers in navigating the healthcare system, including prenatal, delivery, and postpartum care.
    • Provide emotional and informational support to empower patients in making informed decisions about their health.
  • Culturally Competent Care
    • Work collaboratively with translation services to ensure linguistic needs are met.
    • Tailor care to reflect the cultural values and preferences of diverse patient populations.
  • Care Coordination
    • Coordinate access to maternal health resources, including remote monitoring for diabetes, weight, and blood pressure.
    • Partner with community organizations to deliver wrap-around services addressing social determinants of health (e.g., housing, nutrition, transportation).
  • Health Education
    • Deliver educational programs on maternal health topics, such as gestational diabetes, nutrition, and postpartum care.
    • Facilitate group sessions or one-on-one counseling to improve health literacy.
  • Outreach and Program Development
    • Conduct outreach to at-risk populations and build relationships with local community organizations.
    • Collaborate with healthcare providers to develop and implement maternal health improvement initiatives.
  • Data Collection and Analysis
    • Track and report patient outcomes using EMR systems to assess the effectiveness of programs.
    • Analyze disparities in maternal health outcomes to identify areas for improvement.
